Abstract:

The extended running W-plasty technique using the W-plasty principle is a modification of the conventional technique. The use of this technique was utilized for simultaneous reduction of the protuberant labia minora and the redundant clitoris. Twenty-three patients presented to the plastic surgery clinic between 2010 and 2017 with the complaints of protuberant and enlarged labia minora in conjunction with a hypertrophied clitorial hood. The extended running W-plasty was performed in all patients. Surgery was performed under general anesthesia as an outpatient procedure with a range of operative time from 30-45 min. The Likert scale was used to evaluate outcomes. Patients maintained labial length with decreased scarring. Small hematomas occurred in 2 patients and were treated conservatively. One case of wound dehiscence occurred and was also treated conservatively. Patients returned to normal activity 5-7 days postoperatively. The cosmetic outcome of all patients was very satisfactory. The running W-plasty technique is ideal for closure of secondary defects following excision of both the redundant labia minora and clitoral hood, while maintaining length and providing tensionless scars. The technique conserves the original tissues while avoiding over- or under- resection of the labia.

Jingyi Zhao was graduatded from Chinese Academy of Medical Sciences & Peking Union Medical College in July 2017 and  got her doctor’s degree.During the 5 years in Plastic Hospital in Beijing, she learned basic skills of plastic and reconstrucive surgeries, Craniomaxillofacial  surgeries and Cosmetic surgerys.3 SCI articles were published during this period. This manuscript above was one of her clinicle studies, which reported the  use of patient reported outcome measures (PROMS) and patient satisfaction score for the assessment of the outcome of double eyelid surgery in Chinese. It can bu used for blepharoplasty  and other kinds of cosmetic surgeries, which has great  clinical application values, should be popularized.

Abstract:

Statement of the Problem: We previously reported consistent good outcome with the surgical techniques of integration of our modified Z-epicanthoplasty with blepharoplasty. The purpose of this study is to evaluate the surgical outcome from the patient’s perspective using the patient reported outcome measures (PROMS) and patient satisfaction scores. Methodology & Theoretical Orientation: A total of patients (n=180) underwent the surgery between January 2013 to June 2016 were randomly selected to participate in this study. Standardized patient satisfaction form (total score of 40) and validated PROMs questionnaires (total score of 12) were sent to patients for completion. The PROMS consisted of domains which assessed the severity of the scarring, pain, asymmetry, functional and appearances issues. Non-responders were followed up by telephone reminders at 4 weeks later to maximize response rate. Findings: All the patients were females, ranging from 18 to 35 (mean =24) with a response rate of 73.3% (n=132). Majority of patient reported good or excellent outcomes based on the analysis of the results collected from the PROMs. Patients reported minimum or non-visible scarring at both the double eyelid surgical scar (85.6%) and the inner canthus (80.3%) respectively. Functional and appearance issues was minimum with 80.3% reported satisfied with both domains. There is a positive linear correlation between PROMS and patient satisfaction rate (p<0.05). Conclusion & Significance: Integration of our modified Z-epicanthoplasty with blepharoplasty yield good outcome based on the results obtained from the patient report outcome measures, which shown positive linear relationship to the patients’ satisfaction score. To the best of our knowledge, this is the first attempt using the patient reported outcome measures in Asian double eyelid surgery and we advocate its use to standardize the evaluation of post-operative outcomes.

Abstract:

Physiologic changes in a woman’s life, such as childbirth, weight fluctuations, hormonal changes due to aging and menopause, may alter the laxity of the vaginal canal, damage the pelvic floor, and devitalize the mucosal tone of the vaginal wall. These events often lead to the development of genitourinary conditions such as stress urinary incontinence, vaginal atrophy, dryness and physiologic distress affecting a woman’s quality of life, self-confidence, and sexuality. Various treatment modalities were available to manage these indications, varying from invasive vaginal surgery to more benign treatments like topical vaginal hormonal gels or hormone-replacement therapy.

Cosmetic Gynaecology is the latest revolution in the Aesthetic field that changes female external genitalia appearance to look younger and more natural. These are procedures which are cosmetic in nature and some medically necessary to be performed. They are done to either boost up the function or to improve the appearance or both.  A new trend gaining momentum is the advent of energy-based device LASER for vaginal rejuvenation that applies thermal energy to the various layers of the vaginal tissue, stimulating collagen regeneration contracture of elastin fibers, neovascularization, and improved vaginal lubrication. Laser is widely and successfully used for treatments in dermatology and aesthetic medicine. Thus, their application in the vaginal canal for feminine rejuvenation simply represents an expansion of therapeutic indications and hallmarks their versatility and the reactivity of biologic tissues to the beneficial effects of their mechanism of action

Abstract:

Sexuality in Asia has always been regarded in spiritual context since ancient times.  It has always been synonymous with a sacred expression of nature’s life force and mystery of creation. It was believed that sex is a powerful tool of transcendental development and sexual experience was regarded as mystical.

Spiritual sexuality sacred sex aims at the concept of joining energy sources, joining of all the essences of health to attain longevity of life and spiritual advancement.

Ancient Asian people interpreted sex in many ways, main reason were for reproduction and fulfilling physical need of married couple, for complete union of body and soul for intimate partners ,and as a barter to gain monetary or other benefits. There were many aids and ideas to increase the pleasure of sex .Most intrinsic mechanism used were through mind and spiritual path and yogic methods. Kundalini awakening and tantric sexual practices were welcomed. Many ancient texts like Kama sutra from India Ishinpo from Japan gives good guide for sexual practices.

Recent trend of cosmetic gynaecology if practiced in a balanced manner will enhance  sexual pleasure and satisfaction . Cosmetic gynaecology aesthetic gynecology aims at giving outward satisfactory look and also enhances  functional capability. As per the changing times girls and women of today live different ways of life as compared to ancient times. Shape and size of labia which were not considered as of much importance becomes a very important aspect for women athletes and for those wearing swim suits. Vulvovaginal atrophy was one of the commonest problems women used to suffer silently without taking professional help due to lack of awareness and embarrassment. Cosmetic gynaecology has many aspects including labioplasty, vaginal tightening, perineum and pelvic floor repair, vaginal rejuvenation, hymen repair and these are helping women live a satisfactory lifestyle and hereby increasing quality of life.

In recent times quality of sex has fallen and sex has lost its sacredness, its scientific understanding and its simplicity and naturalness. People started  aiming for just for the outward look and temporary physical satisfaction and relaxation and using sex for barter and revenge and coercion and forgetting the very essence of spirit and sacred component of sexuality. Unless we can bring a deep understanding and a harmony to the act of sex, unless we make it spiritual, a better humanity cannot come into being.

We as practicing gynaecologist, as a sexual health provider need to give a message to society to balance the act of cosmetic and spiritual component wisely. Otherwise the very meaning of sexuality and purpose of sexual act will be lost
